If you have spent any time in the GLP-1 conversation in the last twelve months, you have heard the names. Tirzepatide, the second-generation dual-receptor compound that has already reshaped metabolic medicine. Retatrutide, the triple-receptor compound now deep in phase 3 trials with a data profile that has generated a lot of attention. Here is a side-by-side look at what is actually known, what is still incomplete, and how to think about either one.
The short version
Both are peptides designed to activate the gut hormone receptors involved in appetite regulation, blood sugar control, and metabolic signalling. Tirzepatide hits two of those receptors. Retatrutide hits three. More receptors does not automatically mean better outcomes, but in this case the early data suggests retatrutide is producing meaningfully larger weight loss than tirzepatide at the same time point in trials, with a side effect profile that looks broadly comparable.
Tirzepatide, briefly
Tirzepatide activates two receptors:
- GLP-1, the receptor responsible for most of the appetite, satiety and gastric emptying effects.
- GIP, a related receptor with effects on glucose handling and adipose tissue regulation.
The dual mechanism has produced average weight loss of around 22 percent of body weight in the SURMOUNT trials, with strong improvements in HbA1c and cardiovascular risk markers. Tirzepatide is now approved and widely prescribed under brand names including Mounjaro and Zepbound.
Retatrutide, briefly
Retatrutide activates three receptors:
- GLP-1, as above.
- GIP, as above.
- Glucagon, which sounds counterintuitive (glucagon raises blood sugar) but in this context appears to contribute to energy expenditure and fat oxidation.
In the phase 2 TRIUMPH-1 trial, retatrutide produced average weight loss of around 24 percent at 48 weeks. Phase 3 data is still being completed, but the early signal is that the triple-receptor mechanism is producing larger weight loss than the dual-receptor approach over similar timelines.
What the data shows so far
Three things stand out from the early head-to-head conversation:
- Magnitude of weight loss. Retatrutide is producing the largest weight loss of any peptide tested to date in obesity trials. The advantage over tirzepatide is meaningful but not dramatic at most dose levels.
- Speed of weight loss. Both compounds produce most of their weight loss in the first 6 to 9 months, with the curve flattening after that. Retatrutide's curve appears to flatten later, suggesting the additional receptor is doing extra work in the back half of the cycle.
- Side effect profile. Both are dominated by gastrointestinal effects (nausea, occasional vomiting, constipation), which are usually dose-dependent and improve with slower titration. Retatrutide does not yet have a worse side effect profile in published data, although phase 3 will give a clearer picture.
What the data does not yet show
The honest part of the conversation. There are several things we do not yet know:
- Long-term safety beyond 5 years for either compound. The data is good through 2 to 3 years. Beyond that, we are extrapolating.
- Maintenance after stopping. Both compounds tend to be followed by weight regain unless lifestyle is substantially changed. Whether retatrutide's deeper weight loss makes maintenance harder or easier is not yet established.
- Body composition outcomes. Both produce significant fat loss, but they also produce some lean mass loss. The relative balance of fat versus muscle, particularly in older patients, is still being studied.
- Cardiovascular outcomes for retatrutide. The cardiovascular outcome trial is still running. Tirzepatide has positive cardiovascular data. Retatrutide is expected to as well, but the trial has not yet read out.
How to think about either one
Four principles that travel well:
- Start with the foundation. Neither compound is a substitute for adequate sleep, protein intake, resistance training, or stress management. The clients who do well on either are the ones who treat the peptide as one input rather than the whole protocol.
- Resistance training is non-negotiable. Significant weight loss without resistance training and adequate protein produces a worse body composition than weight loss with both. This is true of every weight loss intervention ever studied.
- Titrate slowly. The side effects of both compounds are almost entirely dose-dependent. A slower titration produces a smoother experience and rarely costs anything in eventual outcome.
- Plan the exit. A protocol that does not include a thoughtful exit strategy is incomplete. The conversation about how to wean off and maintain is part of the protocol design, not an afterthought.
Where retatrutide sits in the regulatory picture
As of mid-2026, retatrutide remains an investigational compound. It is not yet approved by the FDA or the TGA. Phase 3 trials are progressing, and an approval submission is widely expected within the next 12 to 18 months. Compounded versions are appearing in some markets, with the regulatory and quality questions that come with that. Anyone considering an investigational compound should be doing so with eyes open and under clinical supervision.
